Bensheim, June 30, 2026 – Despite significant progress in cannabis-based compounding, there remains an unmet need for dosage forms that are both practical for everyday use and amenable to standardisation. Avextra is therefore introducing a novel cannabis granulate based on a balanced THC/CBD extract, designed for the preparation of capsule-based formulations. The granulate forms part of Avextra’s compounding portfolio and serves as a starting material for patient-specific formulations, including options suitable for administration via feeding tubes. Pharmacies can now source the granulate directly and process it locally into individualised capsule formulations containing 2.5 mg or 5 mg THC/CBD. Distribution via pharmaceutical wholesalers is expected from late summer, enabling broader availability, including hospital and long-term care supply.

Development of a new dosage approach

Current treatment with cannabis-based compounded medicines is predominantly based on oily extracts, which may present practical challenges in routine use. At the same time, clinical evidence suggests that the choice of dosage form can influence the overall treatment experience and patient acceptance. In response, Avextra has developed the granulate in close collaboration with physicians and pharmacists to address real-world needs in patient care.

Relevance for patient care

Processing the granulate into capsules in the pharmacy setting allows for clear and reproducible dosing, while facilitating integration into existing treatment and medication regimens. Capsule-based administration may support ease of use and enable therapy to be more readily adapted to individual patient needs in everyday clinical practice.

Clinical context: Saturn

The granulate-based formulation was also evaluated in the non-interventional SATURN study, which investigated different oral dosage forms under real-world conditions. Across multiple indications, treatment with oral cannabis extracts was associated with improvements in symptom burden and quality of life. In addition, a reduction in concomitant medication was observed in a subset of patients, alongside high levels of patient satisfaction with capsule-based formulations.
